What the birth-registration indicator measures
World Bank metadata defines completeness of birth registration as the percentage of children under age five whose births were registered at the time of the survey. The numerator includes children whose birth certificate was seen by the interviewer and children whose mother or caretaker reported that the birth had been registered. The measure is therefore broader than the share of children for whom a physical certificate was shown during the interview.
Birth registration is the official recording of a child’s existence by the state and is an important foundation for legal identity. A high registration percentage, however, does not by itself prove equal access to education, health care, social protection, or every administrative service. A low value also does not identify one cause. Distance to registration offices, administrative procedures, conflict, mobility, legal rules, awareness, and survey timing may all matter. This indicator is descriptive rather than causal.
The median of the 120 latest observations is 87.35%
Giving each of the 120 country observations equal weight produces a median of 87.35% and an unweighted mean of 75.82%. The first quartile is 60.0% and the third quartile is 98.7%. In other words, the middle half of country observations lies between roughly 60% and 98.7%. The mean falls below the median because a group of very low observations extends the lower tail of the distribution.

Fifty-six observations are at least 90%, and 48 are at least 95%. At the other end, 23 observations are below 50% and 47 are below 75%. Twenty-six observations sit in the 99–100% band while eight are below 25%. The distribution therefore combines many near-universal observations with a smaller but important group where registration remained very incomplete at the time of the survey.

Ethiopia has the lowest retained observation at 1.6% in 2016. Somalia is 4.9% in 2020, Zambia 8.2% in 2018, Papua New Guinea 12.1% in 2018, and Angola 13.6% in 2016. Tanzania is 17.7% in 2016 and Chad 21.1% in 2019. The year column is essential: these values describe different survey periods and should not be presented as a current same-year league table.
Ethiopia’s 2016 value, for example, does not establish that it has the lowest rural birth-registration rate in 2026. A present-day assessment would require more recent harmonized observations from UNICEF, the World Bank, or national civil-registration and survey systems. The value of this dataset is to show the last retained observation and the extent of variation, while keeping the age of each observation visible.
Large gaps remain among observations dated 2018–2021
Restricting the table to observations dated 2018 through 2021 leaves 55 countries and economies. Their median is 87.1% and their unweighted mean is 73.33%. The lowest values in that subset are Somalia at 4.9% in 2020, Zambia at 8.2% in 2018, Papua New Guinea at 12.1% in 2018, and Chad at 21.1% in 2019. Mauritania is 29.7% in 2021, making it one of the few observations from the maximum year in the dataset.
Even this narrower subset is not a common-year cross-section. Most observations are from 2018 and 2019; there are ten from 2020 and only four from 2021. It is more accurate to describe the chart as “observations dated 2018–2021” than as a 2021 ranking. The chart is intended to make the low end of the more recent observations easy to see without hiding the time differences.
Mixed observation years are the main limitation

The year distribution makes the limitation visible. There are 21 observations from 2018, 20 from 2019, ten each from 2015, 2016, 2017 and 2020, and only four from 2021. Several countries are represented by much older survey rounds. The fact that 2021 is the maximum year does not make the whole dataset a 2021 dataset.
Latest-observation tables are useful when a series is sparse, but they can mix genuine geographic differences with differences in survey timing. A country with a 2006 observation and one with a 2020 observation are being seen at very different moments. For that reason, every country value should be read together with its year, especially when the numbers are used for policy discussion or cross-country comparisons.
Missing countries are not zero
Only 120 countries and separately reported economies appear in this rural series. Countries such as the Republic of Korea, the United States, and Japan are absent from the retained observations and must not be assigned a value of zero. Missing means that this particular rural birth-registration series does not supply a usable latest value for the country in the table; it does not mean that no children were registered.
Residence-disaggregated indicators depend heavily on household surveys and are not reported by every country on the same schedule. Their coverage can therefore be narrower than the general birth-registration indicator. Treating missing values as zero would collapse two very different situations—no observation and an actual measured value of zero—into one number and would seriously distort a map or ranking.
This article does not calculate a rural–urban gap
The World Bank maintains separate rural and urban series, SP.REG.BRTH.RU.ZS and SP.REG.BRTH.UR.ZS. The country values used here are from the rural series only. No urban percentages are inferred, borrowed from different survey years, or combined to manufacture a rural–urban gap. A valid residence gap requires urban and rural observations for the same country and the same survey year.
That distinction prevents a common interpretation error. A low rural percentage does not prove that the urban percentage is high, and a high rural percentage does not imply that the two residence groups are equal. A direct rural–urban comparison should match the companion series observation by observation and consider the survey design and sampling uncertainty as well as the point estimates.
Data source and calculation method
The statistical definition comes from the World Bank DataBank metadata for SP.REG.BRTH.RU.ZS. The World Bank identifies UNICEF global databases and household surveys such as Demographic and Health Surveys (DHS) and Multiple Indicator Cluster Surveys (MICS) as key sources. Registration completeness includes both certificates seen by interviewers and caregiver reports that a birth has been registered.
Summary statistics are calculated from one latest non-missing observation per country or economy, for 120 observations in total. The mean of 75.82% is therefore an unweighted country mean, not the share of all rural children in the world whose births are registered. The median of 87.35% is the middle country observation, not a population-weighted global rate. Population-weighted analysis would require country-specific numbers of rural children and appropriate survey weights.
